Description
The Michigan Neuropathy Screening Instrument (MNSI) consists of 15 yes-or-no questions that assess feelings in the legs and feet (distal symmetric polyneuropathy). The MNSI also includes a clinical examination that is not included here.

Definition
A questionnaire to determine if a respondent has a history of peripheral neuropathy.
Purpose
Diabetic peripheral neuropathy, also known as distal symmetric polyneuropathy, is a leading complication of both type 1 and type 2 diabetes and is responsible for more than half of all non-traumatic foot amputations in the United States. Incidence of distal symmetric polyneuropathy increases with diabetes duration with up to 50% of those with diabetes having the disease after 25 years (Feldman et al., 1994).
