Protocol - Cognitive Changes
Description
The informant (e.g., caregiver) is asked a series of questions to ascertain changes in the subjects cognition over the past two years. The informant indicates whether the subjects cognition in relation to various situations has improved, stayed the same, or gotten worse. These questions are from the Informant Questionnaire on Cognitive Decline in the Elderly (IQCODE).
Specific Instructions
The introduction refers to the last month or so of the subjects life. The questions can be administered to the informant before that time, perhaps when there is potential cognitive decline.

Now we want you to remember what Rs FIRST NAME was like two years ago and to compare it with what [he/she] is like now. Two years ago was in {CURRENT INTERVIEW YEAR - 2}. I will read situations where Rs FIRST NAME has to use [his/her] memory or intelligence and we would like you to indicate whether this has improved, stayed the same, or gotten worse in that situation over the past two years.
Note the importance of comparing [his/her] present performance with two years ago. So if two years ago Rs FIRST NAME always forgot where [he/she] had left things, and [he/she] still does, then this would be considered "not much change."
Now we want you to remember what Rs FIRST NAME was like [when we interviewed [him/her] in Rs LAST INTERVIEW MONTH, YEAR /two years ago] and to compare it with what [he/she] was like toward the end of [his/her] life, but leaving out the last month or so of [his/her] life. I will read situations where Rs FIRST NAME had to use [his/her] memory or intelligence and we would like you to indicate whether this had improved, stayed the same, or gotten worse in that situation over that time period.

Personnel and Training Required
The interviewer must be trained to conduct personal interviews with individuals from the general population. The interviewer must be trained and found to be competent (i.e., tested by an expert) at the completion of personal interviews. The interviewer should be trained to prompt respondents further if a "dont know" response is provided.
Equipment Needs
The PhenX Working Group acknowledges these questions can be administered in a computerized or noncomputerized format (i.e., paper-and-pencil instrument). Computer software is necessary to develop computer-assisted instruments. The interviewer will require a laptop computer/handheld computer to administer a computer-assisted questionnaire.Requirements
