A 12-item questionnaire that is part of the FACIT© Measurement System that measures spiritual well-being among people with cancer and other chronic illnesses.

Permission to use FACIT© scales can be obtained by completing a simple user’s agreement. Registering will grant access to the scoring and interpretation materials. There is no cost for academic researchers, however, there is a cost at the commercial level. All translations, adaptations, symptom indices, computer programs, and scoring algorithms, and any other related documents of the Functional Assessment of Chronic Illness Therapy (FACIT©) Measurement System, including the Functional Assessment of Cancer Therapy (FACT), are owned and copyrighted by, and the intellectual property of, David Cella, Ph.D. Copyright protection is extended to electronic versions of all FACIT documents and products.
Personnel and Training Required
The FACIT© scales are designed to be self-administered but can be administered by interview format (person-to-person and over the telephone).
The interviewer must be trained to conduct personal interviews with individuals from the general population. The interviewer must be trained and found to be competent (i.e., tested by an expert) at the completion of personal interviews. The interviewer should be trained to prompt respondents further if a “don’t know” response is provided.
Equipment Needs
The PhenX Working Group acknowledges these questions can be administered in a computerized or noncomputerized format (i.e., paper-and-pencil instrument). Computer software is necessary to develop computer-assisted instruments. The interviewer will require a laptop computer/handheld computer to administer a computer-assisted questionnaire.

Selection Rationale
The 12-items were developed based on input of people with cancer or human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS), as well as hospital chaplains and other health-care providers.
